Information For Injured Employees
Idaho Insurance Contact Information
What happens if I am injured on the job?
If the injury is serious or life threatening, go immediately to the nearest hospital emergency room or call 911.
You should ensure you report your injury or illness to your supervisor. Once an on-the-job injury occurs, the Idaho Industrial Commission requires a First Report of Injury or Illness Form (IC1A-!) be filled out and submitted. This form is sent to the Idaho Industrial Commission, the workers' compensation insurance carrier, and one copy is kept for the employer's files. If the First Report of Injury or Illness is not filed within 60 days to the Idaho Industrial Commission and the workers' compensation surety, you could lose all benefits. To get a copy of the form or to find out more, visit the Idaho Industrial Commission's website or call them at (208) 334-6000.
If you are given medical restrictions that should be accommodated once you return to work, be sure to follow them. Also, make sure to make all of your appointments so that your injury or illness is resolved as quickly as possible.
What should I expect when I go to Primary Health Medical Group?
If your employer is setup with Primary Health Medical Group to perform services, simply walk in to any Urgent Care clinic and let the front desk know who your employer is. They will be able to pull up your employer's information, the services we are contracted to provide, and any specific requests they may have asked for. If your employer is not setup with Primary Health Medical Group's Occupational Health Program, there are a few services that we can still provide for you such as work injury care, DOT physicals and immunizations. However, you may be financially responsible for your visit and any associated charges.
Do I always have to go to the same clinic?
No, you can visit any one of NINE Urgent Care clinics for work injury care, as all Primary Health Medical Group clinics are linked through Electronic Health Records. Regardless of which clinic you visit, your complete Primary Health Medical Record will be available to the health care providers. If you are scheduled for a follow-up visit with Dr. Stephen Martinez, Director of Occupational Health, you'll be directed to the clinic most convenient to you (options in both Ada County and Canyon County).
