Patient Information

Whether you have established a health care relationship with one of our Family Doctors, or you are visiting our Urgent Care for the first time, we want your visit experience to be the best it can possibly be. If there is something we can do to make your next visit better, please let us know .

We are continually looking for ways to enhance your clinic experience while providing our community with needed services. In the past few months, we've added additional services like
Adult Immunizations at the State Street Clinic and made select prescriptions available in the clinics (no need to go to the pharmacy when you are feeling bad!). Keep watching for more ways we are creating a better clinic experience for you!

New Patient Information

New patients are always welcome at Primary Health Medical Group's Urgent Care Clinics, Family Doctors, and Speciality Practices. To determine if a Family Doctor or Specialist is accepting new patients, please call their respective office. You can find locations, phone numbers and hours here.

In an effort to help us meet your medical needs, Primary Health Medical Group has compiled some basic information for our first time patients.

For Urgent Care:

  • Primary Health Medical Group's urgent care clinics operate on a first-come, first-serve basis; no appointments are necessary.
  • Please bring along picture identification (such as a driver's license) for the person seeking medical attention. If the patient is a child, please make sure the parent, guardian, or person responsible for the minor has photo identification.
  • If the injured or ill person is covered under a health insurance plan, please bring the insurance card along with you.  Primary Health Medical Group accepts over 2,000 different types of health insurance and we are happy to assist with filing your insurance claim after your visit to one of our clinics.
  • During your first visit, you may be asked questions about your health history so that the medical provider can better assess your condition.
  • Payment is due at the time of service (this includes any co-pay amounts for those covered under health insurance).

For Family Doctors, Pediatricians, OB/GYN, or Specialists:

  • Please arrive at least fifteen minutes prior to your scheduled appointment for your first visit.
  • Please bring picture identification, applicable health insurance information (card) and any completed paperwork that may have been sent to you by our staff prior to your appointment.
  • During your first visit, you may be asked questions about your health history so that the medical provider can better assess your condition.
  • Payment is due at the time of service (this includes any co-pay amounts for those covered under health insurance).

We look forward to assisting you with your healthcare needs at one of our Primary Health Medical Group clinics and welcome the opportunity to turn your first visit into a lifetime healthcare relationship.
