Medical Information Release

To have medical records released, please print he Authorization to Release Medical Information form, complete the form, and fax/mail to any Primary Health Medical Group location.

Primary Health Medical Group will provide each patient  all electronic medical records free of charge once per year. Any requests for paper records (most records prior to 11/1/07) require a fee of $0.20 per page as well as postage (if mailed). Records picked up at any Primary Health Medical Group location will not incur a postage fee. Fees must be pre-paid for medical records.
